Sedgwick County EMS is offering residents a free “File of Life” magnet with a sleeve that holds a patient’s medical information. 

First responders are trained to look for the magnet on the refrigerator during emergencies so they can obtain health information pertinent to the situation.

In addition to offering more space to write your information, this form has questions about primary care physicians, your preferred hospital, medical history and prescription medication(s) you may be taking.
